Consumer Health Data Privacy Policy
This policy is provided for residents of Washington (My Health My Data Act), Nevada (SB 370) and Connecticut, and applies to consumer health data collected through risenmd.com — that is, information that identifies you and relates to your health that is not protected health information handled under HIPAA. Information you provide inside the clinical intake and patient portal is PHI and is governed by our Notice of Privacy Practices, not this policy.
What consumer health data we collect on this website
The website collects no health information. Its only forms are the waitlist and the contact form, which ask for your email address, first name, the program you are interested in, and your state. Because a program of interest could be read as an inference about your health, we treat it as consumer health data. Submitting the form is your consent for us to use it for the one purpose stated below. A first-party analytics beacon records page views and interactions against a random session identifier; it stores no IP address, no device fingerprint and no page URL beyond the route pattern. There are no third-party analytics or advertising tags on this website.

Why we collect it
To tell you when the program you asked about is available in your state, to answer your message, and to understand how the website is used. We do not use it for any other purpose.
Sharing
We do not sell consumer health data and we do not share it for targeted advertising. It is processed by the service providers that operate our website and email (hosting, database and email delivery) under contracts that restrict them to our instructions. We do not share it with affiliates or third parties for their own purposes.
Your rights
You may ask us to confirm whether we collect consumer health data about you, to access it, to delete it, and to withdraw consent to its collection. Email support@risenmd.com; we respond within 45 days and may ask you to verify your email address. You may appeal a decision by replying to our response; we will explain the outcome in writing. If you are not satisfied you may contact your state Attorney General.
Retention
Waitlist entries are kept until the launch sequence for that program completes, then for 12 months; analytics events for 13 months.
Changes and contact
